Obverse description Blue letterpress on white paper with a fine guilloche border framing the entire face. The left and right margins carry the vertical denomination legend 'Fünf Millionen' in large Gothic script. The central text panel, headed 'Stadt Elberfeld' and '5 Millionen Mark' in the upper corners, bears the Notgeld authorisation text in German, with the value 'Fünf Millionen Mark' set in bold Gothic display type over a pale orange underprint vignette. The lower portion gives the issue date 'Elberfeld, 18. August 1923', a serial number, and the facsimile signature of the Oberbürgermeister accompanied by an official violet cachet stamp. To the right a separate panel contains the city arms of Elberfeld — a crowned lion passant within a shield — rendered in blue and orange.

Reverse description Printed in blue on white paper with a bold ornamental border composed of repeating scroll and triskelion motifs. The inscription 'NOTGELD DER STADT ELBERFELD' runs in Roman capitals along both the upper and lower margins. At centre, a large orange underprint vignette depicts a view of the Elberfeld town hall, over which the denomination 'Fünf Millionen Mark' is overprinted in large Gothic blackletter type.
