| Obverse description | Laureate, draped bust of Emperor Caracalla facing right, with short beard visible on the cheek and chin. The imperial effigy is rendered in the characteristic Severan style, with a pronounced brow and strong facial features. The Greek legend encircles the bust along the periphery within a dotted border, identifying the emperor by his full titulature. |

| Reverse description | Asclepius, god of medicine, standing facing with head turned to the left, his serpent-entwined staff (kerykeion) leaning against him. In his outstretched hands he holds small statuettes of the two Nemeses, the patron goddesses of Smyrna, a distinctive iconographic motif linking the homonoia theme to the civic cults of both cities. The reverse legend, distributed across the field and exergue, records the concord (homonoia) between Smyrna and Pergamon and names the presiding strategos Geminus. |
