| Obverse description | Ornate floral and scrollwork border frames a central oval vignette with the Perm city arms — a bear passant on a shield — within a wreath, surmounted by the obligation text of the Perm City Society in Cyrillic. Denomination numeral 5000 appears in circular guilloche panels at both left and right. The lower portion carries a handwritten promissory text in pre-reform Cyrillic script, dated 14 March 1919, with two manuscript signatures of the City Mayor and a Member of the Board. |

| Reverse description | Plain ruled border encloses a text-only composition. The heading ПЕРМСКОЕ ГОРОДСКОЕ САМОУПРАВЛЕНІЕ. is set in large decorative block Cyrillic letters across the upper portion. Below, a paragraph of smaller Cyrillic text in pre-reform orthography sets out the terms of the short-term 6% loan of the Perm City Society, authorised by resolution of the Perm City Duma of 20 December 1917, secured against all movable and immovable property of the city and issued as debt obligations with maturities of 6 to 12 months. A circular stamp impression appears at lower left. |
