| Obverse description | Typeset note printed in black on cream paper with an ornamental border of fine guilloche and foliate corner pieces. The central panel bears the heading ASSYGNACYA in large letters above a decorative cartouche inscribed Assygnacya Skarbowa, beneath which the body text in Polish states the obligation of the General Treasury of the Kingdom of Poland to pay the bearer 500 Złotych with interest at six percent per annum four months from the date of issue. The denomination Złp: 500 appears in the upper right, with the serial number and lit. series designation in the upper left, and the vertical inscription NA ZŁP: PIĘĆSET flanking the left and right margins respectively; below the main text are printed signature lines for the Presiding Minister of the Revenue and Treasury Commission and two Dyrektor Jeneralny, with manuscript signatures present. |

| Reverse description | Plain cream paper reverse with typeset text in Polish arranged in a formal accounting layout. The upper portion carries a printed declaration that Treasury Assignats are secured by tax arrears and other revenues which, by Sejm law, have priority over claims of the Land Credit Society, and that the assignats are accepted in payment of tax and other arrears. Below, printed form fields are provided for issue from the Treasury, receipt date, collector (POBORCA) and controllers (KONTROLLEROWIE), with ruled lines for manuscript completion of capital and interest amounts, a total in Złotych, and a final acknowledgement line reading Powyższą należność w dniu dzisiejszym odebrałem. |
