Beschrijving voorzijde	Black on blue-grey underprint. The upper portion carries an ornate baroque frame with floral and foliate scrollwork at the corners, with denomination numerals '500' in each upper corner medallion and '4%' rosettes at the lower corners. At the top centre, the Imperial double-headed eagle vignette is set within a circular guilloche medallion flanked by laurel branches. The date '1915' appears in rectangular tablets on each side. The central inscription in large Cyrillic letterpress reads 'ПЯТЬСОТЪ РУБЛЕЙ' within a cloud-shaped guilloche panel, above which the title 'БИЛЕТЪ ГОСУДАРСТВЕННАГО КАЗНАЧЕЙСТВА' is printed. Three handwritten signatures appear in the lower register alongside their printed titles, with a serial number below and validity inscription 'Билетъ действителенъ по 1 Февраля 1929 г.' at the foot.

Beschrijving keerzijde	Black letterpress text on a cream ground with a red guilloche lattice watermark-style underprint repeating the numeral '500' across the entire field. The note is framed by a decorative border with corner medallions bearing '500'. The upper half carries the heading 'Положеніе о разрядѣ (серіи) билетовъ Государственнаго Казначейства за № CDLXII (462)' followed by ten numbered regulatory clauses in two columns of small Cyrillic text. Below the text columns, a bold Cyrillic line reads 'ПРОЦЕНТЫ НАЧИСЛЯЮТСЯ СЪ 1 ФЕВРАЛЯ ТЫСЯЧА ДЕВЯТЬСОТЪ ПЯТНАДЦАТАГО ГОДА', and the series designation 'СЕРІЯ CDLXII (ЧЕТЫРЕСТА ШЕСТЬДЕСЯТЪ ВТОРАЯ)' is printed at the foot.
