Beschrijving voorzijde The fifth definitive portrait of Her Majesty Queen Elizabeth II, as modelled by Jody Clark, occupies the central field in right-facing bust form, wearing the George IV State Diadem adorned with pearls and floral motifs, a drop earring visible at the ear. The effigy is rendered in high relief befitting the proof format, with fine hair detail and a composed, mature likeness. The encircling legend reads ELIZABETH II · D · G · REG · F · D · 500 POUNDS · 2019 · distributed around the periphery, with the engraver's initials J.C. incused below the truncation.

Beschrijving keerzijde The reverse presents conjoined busts of Queen Victoria and Prince Albert in left-facing profile, adapted from William Wyon's celebrated 1840 medal design commemorating their marriage. Queen Victoria is depicted with an elaborate upswept coiffure adorned with a laurel sprig, wearing a pearl necklace and drop earring, her bust draped; Prince Albert appears immediately behind, clean-shaven with a short moustache and a plain draped truncation, his bust slightly overlapping hers. A decorative trident-like ornamental device appears to the lower right of the field. The circumferential legend VICTORIA REGINA + ALBERTUS PRINCEPS CONJUX arcs around the upper field within a beaded border, while the date in Roman numerals MDCCCXIX is prominently displayed in the lower exergue.
