| Obverse description | Printed in dark green on pale cream paper, the obverse is dominated by large Gothic blackletter text reading 'Stadt Crefeld' at the top and 'Fünf Mark' across the centre, with the denomination flanking the city's crowned coat of arms set within a guilloche rosette at the midpoint. The underprint consists of a dense, repeating pattern of circular vignettes incorporating the Crefeld arms, the numeral '5', and the dates '1918–1919'. The lower portion carries the issue text, place and date 'Crefeld, den 21. Oktober 1918', the Oberbürgermeister's manuscript signature, and a red letterpress serial number with letter prefix at the foot, with the printer's imprint 'Worms & Lüthgen, Crefeld' in the lower right margin. |

| Reverse description | Printed in blue-grey on cream paper, the reverse centres on an oval intaglio vignette encircled by a beaded border, containing a standing male figure — likely a silk weaver or textile worker, the trade for which Crefeld was renowned — carrying a rolled bolt of cloth over one shoulder and a bag in the other hand, set on a plinth. To either side of the oval, ornate cartouches with scrollwork enclose the denomination '5 MARK' in bold serif lettering. The entire field is covered by a tightly repeated underprint of circular medallions bearing the crowned Crefeld coat of arms, the inscription 'STADT CREFELD', and the dates '1918–1919'; a small crowned shield appears below the central vignette. |
