| Obverse description | Right-facing diademed and draped bust of Queen Elizabeth II, rendered after the fourth definitive portrait by Ian Rank-Broadley, occupying the central field of the oval flan. The legend ELIZABETH II · COOK ISLANDS arcs around the upper periphery, while the denomination 5 DOLLARS appears in the lower field beneath the bust. The mirror-polished proof field provides strong contrast against the finely detailed relief of the effigy. |

Flanking the figure are symmetrical branches of stylised foliage, each set with inlaid red Swarovski crystals simulating roses, totalling seven stones. The papal coat of arms appears in the lower right field.
