Beschrijving voorzijde Central medallion depicts a crowned, armored effigy of Johann III seated facing three-quarters left, holding an upright scepter in his right hand and an orb surmounted by a cross in his left, rendered in bold high relief with elaborate detail to the cuirass and royal robes. The king's long beard and flowing hair emerge beneath a tall arched crown. An inner circle separates the central portrait from a broad outer border densely populated with a continuous frieze of heraldic shields representing the various dominions of the Swedish crown, separated by small crosses. The circular legend runs between the inner beaded ring and the outer rim, reading IOHANNES 3 D G SWECORVM GOTORVAN f RX, identifying Johann III by the grace of God as King of Sweden, the Goths and the Wends.

Beschrijving keerzijde Central device displays the elaborately quartered royal arms of Sweden within a heart-shaped heraldic shield surmounted by a large arched crown, featuring the three crowns of Sweden, the lion of the Folkung dynasty, and additional charges representing the king's territorial claims, all rendered in fine detail. Two inner concentric legends in Latin encircle the shield, conveying a devotional motto. A broad outer border of continuous Latin text fills the rim, reading MISERICORD ET VERITS GVSTOD REG ET ROBOR CLEM THRON EIVS BENE FACIENDO NEMINEM TIMEMVS DEVS PROTECTOR NOSTER, translating as 'Mercy and truth are the guardians of the king and the strength of his throne; doing good, we fear no one, God is our protector.' The entire composition is executed in the robust hammered style characteristic of late sixteenth-century Swedish coinage.
