Obverse description Central field features the coat of arms of Medellín, depicting a fortified castle or city gate with two towers and an arched entrance, surmounted by a crowned figure of the Virgin Mary flanked by foliate ornaments, all rendered within a square frame. Flanking the central shield are two symmetrical laurel branches extending from the lower field. The legend REPUBLICA DE COLOMBIA arcs along the upper periphery in raised Latin lettering, while the commemorative dates 1675 - 1975 appear along the lower border. The entire design is framed by a continuous beaded border following the square with rounded corners format, struck to proof quality with deeply mirrored fields contrasting frosted devices.

Reverse description The central field presents a large, highly detailed spray of orchid blossoms — the cattleya trianae, national flower of Colombia — rendered in high relief with finely modelled petals and stems occupying the majority of the field. The legend TRICENTENARIO DE MEDELLIN curves along the upper periphery in raised Latin lettering. Along the lower border, the fineness inscription 900/1000 ORO appears at centre, flanked on either side by the denomination PESOS 2000 PESOS in bold raised lettering. The design is framed by a continuous beaded border consistent with the obverse, struck to proof standard with deeply contrasting mirrored fields and frosted relief.
