Obverse description Unadorned effigy of King Nicholas I facing left, with a small laurel sprig behind the head, rendered in high relief with fine portraiture detail. A Cyrillic legend curves around the upper periphery identifying the monarch as King and Ruler of Montenegro. The dates 1860–1910, commemorating the fiftieth anniversary of his reign, appear in the lower field flanked by six-pointed stars. The engraver's initials SS are visible in the lower field near the truncation. The overall design is austere and dignified, reflecting the medallic tradition of the period.

Reverse description The crowned and mantled royal arms of Montenegro dominate the centre of the reverse, featuring a double-headed eagle beneath a royal crown with elaborate mantle and tasselled cords to either side. Flanking the shield are sprigs of oak on the left and olive or laurel on the right. The denomination 20 appears in large numerals below the arms, with ПЕРПЕРА inscribed on a curved ribbon beneath, and the date 1910 in the exergue. The Cyrillic legend КРАЉЕВИНА ЦРНА ГОРА (Kingdom of Montenegro) curves around the upper periphery within a beaded border.
