Obverse description Facing bust of Queen Sirikit (Somdej Phra Nang Chao Sirikit Phra Borommarachininat), portrayed with her hair swept back, wearing multiple strands of pearl necklaces and formal royal attire. The effigy is rendered in fine relief occupying the central field. The Thai legend is divided to the left and right of the portrait, reading her full royal title in Thai script.

Reverse description Central device depicts the royal emblem of Queen Sirikit — a crowned ceremonial urn (phumkhao) surmounted by a pointed royal spire, supported by two mythological figures, above an intertwined royal monogram — all rendered in fine relief. Flanking the central emblem are two symmetrical sprigs of foliage and flowers. The denomination '๒๐ บาท' (20 Baht) appears in Thai numerals and script to the lower left and lower right of the central device. The country name 'ประเทศไทย' arches along the upper rim, while two lines of Thai inscription below commemorate the centenary of Thai nursing, dated 12 January BE 2539 (1996).
