Athanasius II ruled Naples simultaneously as both duke and bishop — a concentration of civil and ecclesiastical authority that was highly unusual even by medieval southern Italian standards. He came to power by having his own uncle, Duke Sergiuos II, blinded and exiled, then governed the duchy for over two decades while maintaining nominal Byzantine allegiance as the Aghlabid emirate pressed hard against the region's borders.
