Obverse description The obverse features three conjoined busts in high relief of the Apollo 11 crew in spacesuits, facing forward, with the legend AD ASTRA PER ASPERA arcing across the upper field. Below the busts, in the lower field, the inscription APOLLO 11 appears in large relief lettering alongside the names of the three astronauts and the mission date JULY 20 1969, with the engravers' names CASEY and LOMBARDO incised in the lower field.

Reverse description The reverse depicts a detailed relief of the lunar surface covered with craters, with the Apollo lunar module in orbit above and a small figure of an astronaut on the surface below.
