| 正面描述 | On the obverse of the medal is shown the votive Church of the Holy Mother of Freedom, seen from the east sides to the shrine. Along the edge of the medal above from left to right on a polished background in large Latin letters the text “ZAVJETNA CRKVA SVETA MATI SLOBODE” (`The Votive Church of the Holy Mother of Freedom`) is written. The church was executed in a semi-gloss finish, the natural landscape on which the church was built it is made in a matte finish, and on its base there are indented letters written `ZAGREB`, and under it the year `2009.` |

| 背面描述 | The reverse of the medal shows the center of the liturgical space, the altar relief „Proslava“ (`Celebration`), around which the congregation of believers gathers for the celebration of the mystery of salvation, and the cenotaph with with the names of 15,840 fallen defenders. The central figure is the Risen Christ, depicted as the Victor and the Almighty. On the upper part of the medal above the relief „Proslava“ (`Celebration`) along the edge of the medal from the left on the right is written the text `MARTYRIBUS PRO PATRIA CROATIA IN CHRISTO RESURRECTURIS` (to the martyrs for the homeland of Croatia, who will be resurrected in Christ), and below the `Celebration` relief on the polished surface in the middle is a text in two lines one below the other: „RELJEF POBJEDE“ (`Relief of the Victory`). |
