Obverse description Right-facing draped bust of King John II Casimir Vasa in high relief, wearing a laureate wreath and ornate armour with a jewelled collar. The surrounding legend runs along the inner border of the medal, with the engraver's initials IH visible below the bust truncation.

Reverse description A detailed panoramic view of the city of Toruń (Thorn) set in a broad landscape, with rays of divine light emanating from a radiant cloud at the top of the field and natural clouds to either side. In the exergue area, the date MDCLVIII and XXX DECEMBER are inscribed within a cartouche. The encircling Latin legend commemorates the return of the city to Polish allegiance.
