Obverse description Right-facing bust of Tsar Peter I (Peter the Great) in high relief, depicted with long flowing hair and wearing armor with a fur-trimmed mantle. The engraver's signature DU VIVIER F. appears on the truncation. A circular legend runs along the inner border of the medal.

Reverse description The reverse features an entirely typographic composition in Cyrillic script with no figurative imagery. Inscribed in multiple lines across the flat field are dates and text commemorating the founding of the Leib Guard on 22 August 1700 and the Battle of Narva on 19 November 1700, followed by the anniversary year 1950. Small maker's marks appear at the lower portion of the field.
