Obverse description A gold Maltese-style cross with ornate scrollwork engraving on the four arms, suspended from a plain ring loop. At the centre is a raised shield-shaped escutcheon bearing blue and black vitreous enamel, depicting three five-pointed mullets on a black chief above a golden fleece (a ram passant) on a blue field, derived from the Coat of Arms of Leeds.

Reverse description Plain gold Maltese cross reverse, engraved in three lines with the legend "L.D.L.N.U. / 1902-3 / HOLBECK UNITED F.C." and the recipient's name "R. WOODLER" below, flanked by decorative arrow motifs. Hallmarks in the upper arm include the maker's mark "F&S", fineness mark "9.375", and assay office marks. The maker's name "ATCORINO / BRADFORD" is engraved at the base.
