The emperor's effigy is rendered in high relief, displaying bold facial features typical of Alexandrian tetradrachm portraiture of the Severan period. The encircling Greek imperial legend runs around the obverse field, abbreviated across multiple segments.

Reverse description Sarapis seated on a throne facing left, his left hand resting on a tall sceptre while his right hand is extended forward toward Cerberus, the three-headed guardian of the underworld, who sits attentively at his feet. The god wears his characteristic kalathos (modius) headdress and is draped in flowing robes, rendered in the syncretic Graeco-Egyptian artistic tradition of Alexandria. The regnal year date appears in the left field as L Ϛ (Year 6), corresponding to the sixth year of Septimius Severus's reign in the Egyptian calendar.
