At centre, Attis reclines leftward upon cushions, his legs ritually bound, holding a patera; behind him rises a tree, referencing the Phrygian funerary myth. To the left stands a figure identified as the eponymous hero Kyzikos, depicted in full military dress, facing right, holding a patera and spear. To the right, a musician stands facing left, playing a lyre. In the exergue, a bull walks leftward toward an altar, evoking sacrificial ritual. The encircling legend ΝΕΟΚΟΡΩΝ ΚΥΖΙΚΗΝΩΝ proclaims the city's prestigious status as neokoros, keeper of the imperial cult temple.
