背面描述 Heracles depicted nude, reclining to the left upon a rocky surface, his muscular body rendered in the Hellenistic tradition. His right hand rests upon his knee while his left hand holds a cantharus (drinking vessel), possibly alluding to his labours or divine repose. The reverse legend identifying the issuing city is distributed around the field. The composition reflects the civic pride of Caesarea Germanica and its association with the Heraclean mythological tradition.

Caesarea Germanica was a small Bithynian city whose civic coinage output was modest even by provincial standards, and issues struck for Macrinus — whose reign lasted just fourteen months before he was defeated by forces loyal to the boy-emperor Elagabalus — are correspondingly scarce. The city sat near the Sangarius river in a region that had been refounded under Roman patronage, and its decision to strike for Macrinus at all reflects the logistical lag of provincial mints responding to imperial succession news that could take weeks to arrive overland from Rome.
