Obverse description The central vignette presents a detailed line-art illustration of the Wunderblutkirche (Church of the Miraculous Blood) of Wilsnack, construction of which began in 1384, set within an arched frame with small figures in the foreground. To the left, a circular municipal seal inscribed 'SIGILLVM WILSNACK' is set within an ornate guilloche panel, while to the right a hexagonal church seal reading 'KIRCHEN-SIEGEL WILSNACKSCHES' appears in a matching panel; the denomination '75' appears in large numerals at both upper corners. The lower portion carries the validity date 'Wilsnack gültig bis 20. Juni 1922' at lower left, the denomination in ornate Gothic script 'Fünfundsiebzig Pfennig' at centre, and the facsimile signature of the Bürgermeister alongside 'Der Magistrat' at lower right.

Reverse description The central vignette, rendered in fine black line engraving on a plain ground, illustrates a medieval procession scene captioned 'i. Jahre 1383', in which robed figures carrying a cross-staff and relics move through a landscape with a village in the background, referencing the legend of the miraculous blood of Wilsnack. Flanking the central scene on both sides are tall decorative panels in salmon-pink, composed of stylised foliate and geometric guilloche ornaments with large '75' numerals in white on dark hexagonal cartouches above and 'Pfennig' inscriptions below. A scroll-like panel at the bottom carries a Middle Low German inscription relating to the carrying of the Holy Sacrament.
