Obverse description Central device features the Polish state eagle displayed, with wings spread and head turned to dexter, rendered in high relief against a mirror-polished field. The eagle wears no crown, consistent with the People's Republic era heraldic type. The split date 19-89 appears flanking the eagle's body at mid-field, with the mint mark below. The circular legend POLSKA RZECZPOSPOLITA LUDOWA arcs around the upper periphery, while the denomination ZŁ 5000 ZŁ is inscribed in bold characters along the lower portion of the field.

Reverse description The reverse presents a right-facing portrait bust of Pope John Paul II (Karol Wojtyła), depicted wearing his papal zucchetto and clerical vestments, in a high-relief three-quarter profile. The portrait is set against a finely textured mosaic or basket-weave patterned background that fills the entire field, lending the design a distinctive artistic quality. The vertical inscription JAN PAWEŁ II appears along the left margin, reading upward. The word PRÓBA, denoting trial or pattern status, is inscribed vertically adjacent to the portrait, confirming the coin's experimental strike designation.
