Obverse description Intaglio portrait of Colonel Francisco Bolognesi at right, rendered in fine engraved detail against a multicolour guilloche underprint. The Peruvian national arms appear at centre, flanked by three signature lines below inscribed with their respective titles. The denomination numeral 5000 is printed in large letterpress figures at lower left, with the bank title across the top border.

Reverse description A central intaglio vignette at left illustrates two miners at work inside a mine tunnel, one operating a pneumatic drill and the other loading ore into a rail cart, evoking Peru's mining heritage. A large circular guilloche rosette occupies the right half of the note, reserved for watermark display. Denomination numerals appear in each corner against a fine multicolour guilloche underprint in red, green, and yellow tones.
