| Obverse description | The Imperial double-headed eagle arms vignette is positioned at centre, flanked by the denomination '5000 р.' printed twice in red. The upper portion carries the main title inscription in Cyrillic within a lightly ornamented border, with issue and repayment dates (15 August 1915 and 15 February 1916) noted in dedicated fields.
