Obverse description The Bulgarian state coat of arms is centrally placed within a guilloche underprint border. Below the arms, the denomination inscription «ПЕТ ХИЛЯДИ ЛЕВА» is rendered in bold Cyrillic lettering. The overall composition is framed by ornamental guilloche patterns with the issuing bank name and place of issue inscribed across the note.

Reverse description The large numeral «5000» occupies the central field, flanked by intricate guilloche scrollwork. The bilingual denomination text «ПЕТ ХИЛЯДИ ЛЕВА» in Cyrillic and «BANQUE NATIONALE DE BULGARIE» in Latin script are inscribed across the central panel. A fine lathe-work border frames the entire reverse design.
