At centre-left, a large optically variable ink element is integrated into the design, accompanied by the Millennium commemorative inscription "ROK 2000 / BIMILÉNIUM / ANNO DOMINI / MM" within a dotted circular border. The denomination "5000" appears in large numerals at upper centre, with the vertical legend "PÄŤTISÍC SLOVENSKÝCH KORÚN" running along the left field. |

The 5000 Sk Millennium note was a commemorative issue rather than a replacement for the standard 5000 Sk series — Slovakia's highest denomination, and one of the few Slovak notes designed specifically to mark a calendar event rather than fill a transactional need. Giesecke & Devrient in Leipzig handled production, as they did for much of the Slovak koruna series throughout the 1990s.
Václav Fajt's engraving work is the technical highlight here. Fajt was among the more accomplished Czech engravers active in this period, and the intaglio quality on this issue is noticeably finer than on earlier high-denomination Slovak notes.
