Obverse description The Serbian state coat of arms occupies the central field, depicting a double-headed eagle surmounted by a royal crown, with a shield bearing the Serbian cross and four firesteels on its breast. The bicentennial years '1804' and '2004' flank the crowned eagle. A circular legend in both Cyrillic and Latin script reads 'РЕПУБЛИКА СРБИЈА · РЕПУБЛИКА СРБИЈА' around the upper periphery, while the denomination '5000 ДИНАРА · DINARA' appears along the lower border.

Reverse description A dignified portrait bust of Đorđe Petrović, known as Karađorđe, leader of the First Serbian Uprising, rendered in high relief and facing slightly to the right. The effigy is depicted in period military attire befitting his role as commander. A Cyrillic inscription identifying the subject by name arcs around the portrait, accompanied by the dates of his birth and death.
