背面描述 The reverse presents a richly detailed high-relief depiction of the Iveron Icon of the Most Holy Theotokos, rendered in the traditional Byzantine iconographic style. The enthroned Mother of God is shown in three-quarter view, her right arm extended presenting the Christ Child, who is seated upon her left arm and rendered with a halo; both figures display finely chased drapery in frosted silver. The icon's golden oklad (riza) border is highlighted with gold plating, featuring repeating floral and foliate motifs in the Orthodox decorative tradition. The surrounding border incorporates smaller devotional medallions of saints in relief set against a dark background, framing the central icon in the manner of a traditional icon cover. The inscription 'ІВЕРСКАЯ' appears in a cartouche at the base of the design, identifying the icon.

Belarus has issued an extensive series of large-format silver religious coins since the mid-2000s, part of a deliberate program to commemorate Orthodox Christian heritage tied closely to the Belarusian national identity promoted under Lukashenko's government. The Iveron icon holds particular significance in Eastern Orthodoxy — the original, housed at the Iviron Monastery on Mount Athos since at least the 10th century, is venerated as protective of the faithful and was brought to Moscow in 1648 at the personal request of Tsar Alexis.

The 500-rouble denomination placed this well outside ordinary circulation from the outset — a collector instrument, not currency.
