Obverse description Left-facing bust of Pope John XXIII wearing the pontifical vestments, including an ornately embroidered cope visible at the truncation, and the white zucchetto. The effigy occupies the central field within a raised inner border. The circular legend reads IOANNES · XXIII · PONT · MAX along the upper arc, with AN · I (indicating the first year of his pontificate) disposed along the lower arc flanking a small star mintmark.

Reverse description Central field features the papal coat of arms of Pope John XXIII: a vertically oriented heraldic shield surmounted by the papal tiara and flanked by two crossed keys bound with a cord, all rendered in fine relief. The denomination LIRE 500 is inscribed in two parts flanking the lower portion of the shield. The circular legend CITTÀ DEL VATICANO runs along the upper arc, while the date 1959, flanked by two small rosette ornaments, appears in the lower exergual area within a raised inner border.
