Obverse description Central design features a stylized cascade of waving flags rendered in a modernist, geometric style, with the Greek national flag prominently displayed among them in the upper right portion of the field. The denomination '500 ΔΡΧ' appears in large raised characters to the right of centre. The legend 'ΕΛΛΗΝΙΚΗ ΔΗΜΟΚΡΑΤΙΑ' curves along the upper periphery in Greek script. A small mint mark appears in the lower right field.

Greece hosted the 1991 Mediterranean Games in Athens and Thessaloniki — the first time the country had hosted the event since the Games' inaugural edition in Alexandria in 1951. This commemorative issue was released in conjunction with that hosting, part of a broader Greek commemorative silver program that accelerated sharply through the late 1980s and early 1990s as the Bank of Greece leaned heavily on collector coinage for foreign currency revenue.
