Obverse description Right-facing effigy of Queen Elizabeth II at approximately 77 years of age, rendered in the fourth definitive portrait by Susanna Blunt. The Queen is depicted bare-headed, wearing a necklace and drop earrings. The surrounding legend reads ELIZABETH II D·G·REGINA in raised Latin capitals along the upper periphery, with the designer's initials present in the field.

Reverse description A highly detailed composition of three sugar maple leaves (Acer saccharum) rendered in high relief against a deeply mirrored proof field. The central leaf is large and prominently raised, displaying finely engraved vein detail, flanked symmetrically by two smaller leaves on either side, their stems crossing at the centre. The legend CANADA arcs along the upper periphery, while 500 DOLLARS and the date 2012 appear along the lower border. The engraver's initials LN are visible in the lower right field.
