The denomination PENCE is inscribed at the base of the coin, while the legend NHS repeated multiple times encircles the entire design in the outer border, referencing the National Health Service. The NHS 50th Anniversary 50p was issued in 1998 to mark the founding of the National Health Service on 5 July 1948 — Aneurin Bevan's defining political achievement, pushed through against fierce opposition from the British Medical Association, which initially urged doctors to boycott the scheme entirely. The gold proof struck in that year was among the first 50p issues produced in 22-carat gold at this specification.
The extended date range through 2009 reflects ongoing restrikes for collector sets rather than any commemorative reissue.
