Obverse description Blue intaglio print on multicolored guilloche underprint. The central field carries the issuer name BANK POLSKI in a bold banner at top, with the denomination PIĘĆ ZŁOTYCH in large lettering below, and the date and place of issue WARSZAWA DNIA 15 SIERPNIA 1939 R. in the middle. Three signature lines for PREZES, NACZELNY DYREKTOR, and GŁÓWNY SKARBNIK appear beneath, with an alphanumeric serial number at lower left. To the right, an oval vignette presents a portrait of a young woman in traditional Polish folk headdress adorned with flowers, rendered in fine intaglio engraving.

Reverse description Printed entirely in blue, the reverse is dominated by an ornate symmetrical design of interlocking guilloché patterns, scrollwork, and stylized foliate ornaments. A large numeral 5 is set within a circular guilloche at center top, flanked by two BB monogram cartouches, above a ribbon scroll bearing PIĘĆ ZŁOTYCH in bold lettering. A lower text panel carries the legal tender declaration, and the printer's imprint BRADBURY, WILKINSON & CO. LD NEW MALDEN, SURREY, ENGLAND appears at the very bottom margin.
