Obverse description The fifth and final definitive effigy of Queen Elizabeth II, sculpted by Jody Clark, occupies the centre of the obverse, depicting the Queen in right-facing profile bust, crowned with the George IV State Diadem and wearing a drop earring. The portrait is rendered in high relief with finely detailed hair and crown ornamentation. The surrounding legend reads ELIZABETH II · D · G · REG · F · D · 5 POUNDS · 2022, disposed continuously around the rim, with the engraver's initials J.C. incuse below the truncation of the bust.

Reverse description The reverse, designed by P J Lynch, commemorates the Queen's charitable work and patronage. Arranged in an arc across the upper field is a sequence of five coins depicting the successive official portrait effigies of Elizabeth II used on British coinage throughout her reign, progressing from the youthful Mary Gillick portrait to the Jody Clark fifth portrait. Dominating the lower field is a large facsimile of the Queen's personal royal signature, 'Elizabeth R', rendered in flowing cursive relief. The arc of coins is separated from the polished field below by a subtle decorative line, and the reverse legend HER CHARITY AND HER PATRONAGE appears along the upper rim, with the designer's initials PJL at the lower right.
