Obverse description The fifth and final definitive effigy of Queen Elizabeth II, sculpted by Jody Clark, occupies the centre of the obverse, depicting the monarch as a right-facing crowned bust wearing the George IV State Diadem and a drop earring. The portrait is rendered with fine detail in the hair and crown jewels. The circumferential legend reads ELIZABETH II · D · G · REG · F · D · 5 POUNDS · 2022 · in incuse lettering, with the engraver's initials J.C. incused beneath the truncation of the bust.

Reverse description The reverse, designed by P J Lynch, features an array of British state honours and decorations suspended from ribbons across the upper field, including the Victoria Cross, the George Cross, the Order of the British Empire and other distinguished medals rendered in fine relief. Dominating the lower two-thirds of the field is a large facsimile of Queen Elizabeth II's royal cypher signature, 'Elizabeth R', engraved in flowing script across the polished field. The curved legend THE BESTOWING OF HER HONOURS arcs along the upper periphery, with the engraver's initials PJL placed to the lower right.

Released as part of a broader commemorative program issued in the final months of Elizabeth II's life, this piece appeared while the Queen was still on the throne — she died in September 2022, making the entire "Queen's Reign" series a de facto retrospective rather than the celebratory issue the Mint had intended. The honours system it references dates to medieval origins but was substantially reorganized under the Order of the British Empire, established by George V in 1917 specifically to recognize civilian contributions during the First World War.
