Obverse description The obverse features the fifth definitive effigy of Queen Elizabeth II, designed by Jody Clark, depicted in right-facing profile with a jewelled crown, drop earring, and necklace. The portrait is rendered in high relief with fine detail in the hair and crown ornamentation. The surrounding legend reads ELIZABETH II·D·G·REG·F·D·5 POUNDS·2021, separated by raised dots and arranged around the full circumference of the coin. The engraver's initials J.C appear incuse below the truncation of the bust. The field is plain and unadorned, lending prominence to the royal effigy.

Reverse description The reverse, designed by Gary Breeze, presents a typographic composition filling the field with the commemorative inscription in bold, evenly spaced capital lettering across five lines: AT THE GOING DOWN OF THE SUN & IN THE MORNING WE WILL REMEMBER THEM, drawn from Laurence Binyon's 1914 ode 'For the Fallen.' In the lower right quadrant, a cluster of three red Flanders poppies rendered in vivid colour printing rises from slender green stems, serving as the principal emblem of remembrance. The designer's initials GB appear incuse at lower right, adjacent to the floral motif. The reverse field is otherwise unadorned, allowing the inscription and coloured poppies to carry the full commemorative weight of the design.
