Obverse description A pearl border frames the outer rim, enclosing a circular legend in raised Latin lettering. Within a secondary inner pearl circle, the municipal coat of arms of Schwerin an der Warthe is centrally placed, depicting a rampant lion on a divided shield surmounted by a mural crown. The founding date 1722 appears flanking the shield on either side within the field, referencing the town's charter year.

Reverse description A pearl border runs along the outer rim, within which the circular legend KLEINGELDERSATZMARKE (small change substitute token) is inscribed in raised Latin lettering. An inner rope-twist circle encloses the large numeral '5' centered prominently in the field, denoting the denomination. Three five-pointed stars appear in the lower portion of the annular space between the rope circle and the legend, serving as decorative separators.
