Obverse description A circular cameo device is set at the center of the rectangular planchet, bearing the right-facing effigy of Queen Elizabeth II in the Ian Rank-Broadley portrait style. The legend ELIZABETH II arcs along the left, NIUE along the upper portion, and FIVE DOLLARS along the right of the circular border, all rendered in raised Latin lettering against a polished field. The date 2013 appears below the effigy within the circular device, and the inscription 80g 999 FINE SILVER is engraved in the lower margin of the rectangular field beneath the circular cameo. A beaded border frames the circular design element, contrasting with the frosted rectangular surround.

Reverse description The rectangular reverse reproduces, in full color high-relief, a close detail from Michelangelo's celebrated fresco The Creation of Adam, originally painted on the ceiling of the Sistine Chapel (1508–1512). The composition focuses on the reclining figure of Adam, rendered in warm ochre and earth tones, his torso and limbs depicted with naturalistic anatomical detail and visible surface texture suggesting aged fresco. The fingertips of Adam's left hand are visible at the upper left edge of the design, extending toward the divine touch, while his right hand rests at the lower right. The image is bordered by a plain rectangular edge with a narrow raised rim, emphasizing the painterly quality of the colorized artwork.
