Obverse description Crowned and draped effigy of Queen Elizabeth II facing right, after the fourth definitive portrait by Ian Rank-Broadley, wearing the Diamond Diadem and drop earrings. The legend ELIZABETH II arcs along the left field, 5 DOLLARS along the upper right, and COOK ISLANDS along the right. The engraver's initials IRB appear below the truncation, with the date 2014 in the lower field.

Reverse description The reverse features a large yin-yang symbol occupying the entire field, with the two interlocking teardrop sections each containing a stylised animal rendered in a bold tribal art style. The upper-left light section depicts a caribou in dynamic pose, its antlers and body formed by flowing, interlocking curves. The lower-right dark section features a wolf in a crouching stance, similarly rendered in tribal graphic style.
