Obverse description Within a circular recessed medallion set into the rectangular planchet, the right-facing effigy of Queen Elizabeth II is depicted in a diademed and draped portrait. The legend ELIZABETH II · NIUE · FIVE DOLLARS arcs around the periphery of the medallion, with the date 2012 positioned in the lower exergue. Below the medallion, in the lower field of the rectangular surface, the inscription 100g · 999 FINE SILVER appears in incuse lettering.

Reverse description A full-color reproduction of a detail from Leonardo da Vinci's celebrated mural 'The Last Supper' (c. 1495–1498) is applied to the rectangular reverse field. The scene depicts the figures of the Apostles Jude Thaddaeus and Simon the Zealot, shown in animated conversation at the supper table, rendered in the warm tonal palette of the original Milanese composition. The background features architectural elements characteristic of the Renaissance setting, and the figures are clothed in flowing robes. The highly detailed color printing faithfully replicates the texture and chiaroscuro of the original fresco.
