Obverse description Crowned and draped right-facing effigy of Queen Elizabeth II after the fourth definitive portrait by Ian Rank-Broadley, occupying the central field. The Queen is depicted wearing the Girls of Great Britain and Ireland tiara. The engraver's initials 'IRB' appear below the truncation of the bust. The circumferential legend reads 'ELIZABETH II' to the left and 'NIUE FIVE DOLLARS' to the right, with the date '2016' inscribed along the lower rim.

Reverse description High-relief depiction of two cupped hands rendered in antique silver finish, cradling a boldly sculpted heart applied in vivid red colour enamel at the centre of the field. The hands are shown from the front, fingers interlaced at the base, conveying a gesture of offering or devotion. The romantic motif is executed in a naturalistic, sculptural style consistent with the high-relief striking technique. The inscription 'I GIVE YOU MY HEART' is incused along the lower arc of the field beneath the hands.
