Obverse description Right-facing diademed and draped effigy of Queen Elizabeth II, modelled by Ian Rank-Broadley, whose initials IRB appear beneath the truncation of the neck. The legend ELIZABETH II arcs along the upper curved edge of this arch-shaped flan, while COOK ISLANDS and 5 DOLLARS are inscribed in two lines across the flat lower edge.

Reverse description Central design depicts a partially opened Gothic cathedral door set within an ornate stone archway, symbolising the Holy Door. Radiating from a heart-shaped inset crystal at the centre, gilt rays of light fan outward across the field, with a pair of gilt praying hands rendered in high relief at lower right. Three applied pink gemstones are arranged vertically along the right field, above a small anchor symbol. The title ANNUS FIDEI and the date range OCTOBER 2012 - OCTOBER 2013 are inscribed along the upper right, with the year 2013 in the lower left exergue.
