Descrizione del dritto	Bare-headed right-facing effigy of King Charles III after the portrait by Dominic Taddei, rendered in high relief against a frosted field within the heart-shaped flan. The engraver's initials 'DT' appear beneath the truncation of the bust. The legend 'CHARLES III' curves along the upper right periphery, while '5 DOLLARS' arcs along the upper left, and 'COOK ISLANDS' is inscribed along the lower arc, all separated by raised dot stops.

Descrizione del rovescio	The heart-shaped reverse features a richly coloured floral and botanical composition set against a polychrome background of pink, red, and golden-yellow blossom motifs applied in colour. Three principal flower designs occupy the field: at upper left, a finely engraved daisy-style bloom inlaid with a faceted amber-toned Swarovski crystal at its centre; at lower left, a stylised rosette with marquise-cut petals centering a vivid red Swarovski crystal; and at lower right, a large circular bezel setting housing a prominent deep-red faceted Swarovski crystal. A delicate dragonfly with coloured wings hovers among the blossoms at centre right, accompanied by a silver leaf sprig. The date '2026' appears in small characters near the lower left edge of the field.
