| Opis awersu | Printed in blue on plain paper, the note is framed by an ornate floral and fan-motif border with the repeating initials C.M.M. at the corners. The Portuguese Coat of Arms is set within a circular guilloche vignette at top centre, flanked by foliate ornaments and surmounted by a ribbon scroll bearing the issuer's name. To the lower left, the numeral '5' is set within an elaborate rosette of interlocking geometric and floral lathe-work, with the denomination text 'VALE' above and 'CENTAVOS' below; the series designation '3.a Serie' and the manuscript signature of the President of the Executive Committee appear to the right. |

| Opis rewersu | Printed in brown on plain paper, the reverse is enclosed by the same fan and floral border with repeating C.M.M. corner initials as the obverse. The central vignette is a finely engraved panoramic landscape view of Caldas de Monchique, showing the hillside spa village with whitewashed buildings, a church tower, tall pine trees and dense vegetation rendered in precise line work. At top centre, the numeral '5' is placed within a double-ringed circle flanked by the denomination text 'VALE' and 'CENTAVOS', while a rectangular cartouche at the base of the vignette carries the location caption. |
