A decorative foliate border frames the note on all four sides, with a vertical side panel at right bearing the denomination numeral '5' and the word 'Millionen' in large blue Gothic (Fraktur) script. The denomination 'Fünf Millionen Mark' is rendered in large green Gothic lettering at centre, above a two-line redemption clause in black Gothic script; the issuance place and date 'Essen, den 15. August 1923' appear at lower left, with the issuing authority's facsimile signature and the designation 'Der Landrat I.V.' at lower right. Series letter 'A' and a serial number appear at upper left and upper right respectively. |

At centre, a rectangular vignette presents a photographic-style letterpress view of the Kreis-Säuglingsheim (district infant home) at Kettwig an der Ruhr, captioned below the image in small roman type. Flanking the central vignette on both left and right are three vertical ornamental panels each containing an Art Nouveau foliate cartouche. At the top, the full denomination 'Fünf Millionen Mark' is set in large Gothic script, while at the bottom the numeric value 'MARK 5000000 MARK' is rendered in bold sans-serif capitals. |
