Obverse description Right-facing effigy of Queen Elizabeth II after the Ian Rank-Broadley portrait, depicting the Queen as a mature sovereign wearing the Girls of Great Britain and Ireland tiara. The truncated draped bust is shown in high relief against a mirror-polished field. The legend ELIZABETH II arcs along the left rim, while COOK ISLANDS curves along the right rim. The coin is bordered by a fine beaded inner circle.

Reverse description The large numeral '4' dominates the centre of the reverse in the traditional Maundy style, flanked by the split date 20 and 02 on either side. Above the numeral, a Tudor-style royal crown surmounts the design, with the word PENCE visible on a scroll beneath it. The central motif is framed by two symmetrical sprays of olive or laurel branches tied at the base, in direct homage to the classic British Maundy coinage reverse design. A fine beaded border encircles the entire composition.
