| Obverse description | Plain typeset cheque-style note with a simple rectangular border. The large red numeral '25' dominates the centre as the value indicator, flanked on either side by the denomination in Cyrillic script reading 'Двадцать пять рублей'. A circular State Bank branch seal is struck at the upper left, and three manuscript signature lines for the Manager (Управляющий), Controller (Контролёр), and Cashier (Кассир) appear at the foot of the note. |

Two paragraphs of Cyrillic text in small letterpress type set out the legal basis and guarantees of the note, citing the resolution of the Sakhalin Regional Executive Committee dated 14 March 1918. A final line warns that counterfeiting is prosecuted by law. |
| Reverse lettering | Этотъ чекъ согласно постановленiю Сахалинскаго Областного Исполнительнаго Комитета Совѣта рабочихъ, солдатскихъ и крестьянскихъ депутатовъ и Самоуправленiй отъ 14 марта 1918 го безсроченъ и, согласно тому же постановленiю, имѣетъ хожденiе въ предѣлахъ Сахалинской области наравнѣ съ кредитными билетами, — слѣдовательно обязателенъ къ прiему во всѣ платежи. Выпущенные Николаевскимъ на Амурѣ Отдѣленiемъ Государственнаго Банка чеки обезпечены хранящимся въ Отдѣленiи Банка неприкосновеннымъ запасомъ кредитныхъ билетовъ 1.000 рублеваго достоинства и въ случаѣ предъявленiя чековъ на достаточную сумму на эти (тысячерублевые) билеты безпрепятственно обмѣниваются. Подѣлка преслѣдуется закономъ. |
