Obverse description An uncut consumer ration card sheet of 28 individual coupons arranged in a grid pattern, printed in pink and yellow tones over a guilloche underprint on each coupon, with denominations of 1, 3, 5, 10, and 25 roubles inscribed in Belarusian alongside the issuing authority designation. A central register block carries the consumer card header, handwritten registration fields for surname, issuing authority, administrator, and chief accountant, together with an official circular ink stamp impression. An anti-counterfeiting notice appears within the register block, the whole sheet consistent with wartime-style consumer rationing document production.

Reverse description Plain unprinted white reverse bearing faint horizontal ruled lines across the full sheet, consistent with standard ration booklet paper stock. A circular administrative ink stamp impression from the obverse has bled through, visible near the centre of the sheet.
