Obverse description The Polish state eagle — a crowned, spread-winged white eagle facing right — occupies the centre of the field in high relief against a mirror-polished proof background. The date '1995' is divided by the eagle's talons in the lower centre field, with the denomination '20' in large numerals below and the currency abbreviation 'ZŁ' flanking it on either side, all separated by decorative dot ornaments. The circular legend 'RZECZPOSPOLITA POLSKA' arcs along the upper rim in Latin characters. The design follows the standard Third Republic heraldic type used for Polish commemorative silver issues of the 1990s.

Płock Voivodeship's 500th anniversary in 1995 marked the administrative division established under the 1495 reorganization of Polish territorial governance — though Płock itself had served as a royal capital under the Piast dynasty centuries earlier, briefly hosting the Polish court before Kraków's consolidation as the primary seat of power. The commemorative program that year was unusually ambitious, with Mennica Polska producing several silver issues targeting the collector market during Poland's post-communist numismatic expansion.

Mintage on this type was limited to 20,000 pieces.
